Subject: Very, Very Poor Quality Scan

Scan (at least in the .pdf) is of extremely poor quality. Many pages are blurred or distorted, and numerous pages of the Index are so bad as to be completely illegible and useless. Surely it would be possible to obtain a better scan, and to institute more effective quality control, to avoid posting such poor versions.
